MyIntealth Applicant Account Establishment & Identity Verification
The first step to start using MyIntealth is to complete a request to establish an account. The request requires you to submit some basic personal information, details about your medical education, and a digital photograph and scanned image of your passport. If applicable, you’ll also be asked to provide details about your medical registration/licensure and information on organizations to which you may be applying for registration/licensure, employment, or other appointment.

Before you begin your account establishment request, please note the following:
- There is a fee for requesting to establish a MyIntealth account that is due at the time of submission. For more information on the schedule of fees encountered most frequently for ECFMG services, see the Fees page.
- Your eligibility for each service will be determined as part of each service request, not as part of your MyIntealth account establishment request.
- Once you begin your request, it will be saved for 14 days. If you do not submit your request within 14 days, it will be deleted, and you will need to begin a new request if you wish to establish an account.
- You are responsible for reading and becoming familiar with the Policies and Procedures Regarding Irregular Behavior before you request an account.
What You’ll Need
To complete the MyIntealth account establishment request, you will need:
- a full-color photograph of your face (see the photograph section below for more details);
- a scanned, full-color copy of the photograph page of your current, unexpired passport (see the passport section below for more details);
- information about your medical education, including your medical school identification number (if applicable) and attendance and degree issue dates;
- your medical licensure information (if applicable), including any identification numbers;
- your United States Medical Licensing Examination (USMLE) Identification Number (if you have one);
- your ECFMG International Credentials Services (EICS) Identification Number (if you have one); and
- your ECFMG Electronic Portfolio of International Credentials (EPIC) Identification Number (if you have one).
Photograph
You will be required to upload a photograph of your face as part of your request to establish a MyIntealth account. Depending on the additional services you request, Intealth may include this photograph on documentation that is shared with other organizations as part of fulfilling your service requests. As such, the photograph you submit should show a true likeness of you in a professional manner. The source of the photograph should be either a digital camera or a print photograph scanned into a digital format.
Your photograph must:
- Show a clear, front view of your full face only. Your face should be in the middle of the photograph, and your expression should be natural with your eyes open and looking directly ahead.
- Be in color.
- Have a plain, light-colored background.
- Be of good quality. The photograph should not be blurry and should not be too dark or too light.
- Be current. The photograph must have been taken within six months of the date you submit it.
- Be of a high-enough resolution to print well on your Intealth Identification Form and other documentation.
- Be in JPG/JPEG file format.
- Have a file size of 10 MB or less.
If your photograph does not meet these requirements, it will be rejected, and you will need to upload an acceptable photograph.

Passport
You will be required to upload an image of your current, unexpired passport as part of your request to establish a MyIntealth account. Passports are used by Intealth for identity verification only.
The image of your passport must:
- Show your passport at actual size.
- Be clear and legible.
- Be in color.
- Contain the page with your name and photo.
- Include the passport expiration date. If the image of the page with your name and photo does not include the expiration date, you will also need to upload an image of the page that includes your passport expiration date.
- Include the section of your passport that is in Latin characters. If the image of the page with your name and photo is not in Latin characters, you will also need to upload an image of the section of your passport that is in Latin characters.
- Match the name, gender, and date of birth you enter as part of your account establishment request. If the information you submit does not match exactly the same information in your passport, we will change the name, gender, and date of birth to match the information in your passport.
- Include a complete, unobstructed view of the machine-readable zone (MRZ code), if the MRZ is present on your passport.
- Be in JPG/JPEG file format.
- Have a file size of 10 MB or less.
If your passport image does not meet these requirements, it will be rejected, and you will need to upload an acceptable passport image.

Verifying Your Identity – Intealth Identification Form
Once your initial account request has been processed, you will be notified to return to MyIntealth and provided with instructions for the final step in the account establishment process — completion of the Intealth Identification Form (IIF). You’ll be asked to review your personalized IIF and then release it for notarization by NotaryCam, which provides access to live online sessions with professionally licensed and certified U.S. notaries. You will have six months from the date your personalized IIF is created to complete the online notarization process. If you do not complete the online notarization process within this time frame, your identification form will no longer be valid, and you will be required to re-submit a current photo to obtain a new identification form.
Once your form has been notarized and accepted by Intealth, you will have access to request services available through MyIntealth.
